What to Expect?

When you arrive for your first CellSound treatment, standardized photographs will be taken with a green screen, as measurements can be calculated by the computer. Electrodes will be placed on the skin, so please ensure that the areas treated are clean and shaven (jawline, sideburn, abdomen). The electrodes will cause the muscles on one side of the face or body to contract, while ultrasound gel is applied to the skin surface on other side. This ultrasound applicator is passed over the skin, safely delivering warm ultrasound energy to the deeper tissues to stimulate blood flow, clear lymphatics, and stimulate collagen. A separate applicator can be used to target unwanted fat cells to release their lipid content, which can be readily available for the body to use and eliminated. Any exercise performed in the following days becomes more efficient at reducing this fat, while the overlying skin remodels over the next three months.

Skin Tightening

Skin tightening using Forma (RF) is a non-invasive and highly effective treatment designed to improve skin laxity and firmness. This radiofrequency technology delivers controlled heat energy to the deep layers of the skin. Neck Tightening

Neck skin tightening can be achieved using Morpheus 8 radiofrequency (RF) microneedling, CellSound ultrasound treatments, and Hollywood Spectra laser toning. These treatments can be combined with off-label Botox treatments for the platysmal bands hyper-diluted hyaluronic acid fillers, collagen stimulating fillers, and thread lifting to achieve a more smooth and youthful jawline and a more graceful neck.

Body Contouring & Fat Reduction

We all have stubborn pockets of fat or loose skin that we would like to improve without the need for liposuction or surgery.  CellSound is an innovative technology to target and disrupt fat cells, leading to their gradual elimination from the body. Unlike liposuction or abdominoplasty which require puncturing the wound and risk for infection or dimpling, CellSound offers a non-invasive fat reduction method that is painless and does not involve and needles, incisions or anesthesia.  This allows your to achieve your body contouring goals without pain or downtime.

One readily available treatment option for non-invasive fat reduction is cryolipolysis to freeze the fat, commonly known as CoolSculpting.  However, this treatment can result in paresthesia or nerve pain or unwanted paradoxical lipodystrophy which causes an increase in unwanted fat, which then must be treated with liposuction.  Other treatments such as laser lipolysis or radiofrequency microneedling also require puncturing the skin.

CellSound is the preferred technique at Dynasty Dermatology, as its work by selectively targeting fat cells, triggering a natural process of fat elimination through the body’s lymphatic system.  Non-invasive fat reduction treatments can be performed on the chin, abdomen, thighs, flanks, and arms to help create a sculpted and more contoured look for trouble spots all over the body.

CellSound works by relying on the non-invasive application based on the synergistic combination of two safe technologies:

• Low-intensity of non-focused ultrasonic resonant technology.
• Electric current fields to change the impedance of the skin.

The CellSound System’s ultrasound transducer is applied to the treatment area and creates mechanical stress leading to cell resonance, which leads to the release of free fatty acids.  A hypersonic non-focused wave of low intensity is transmitted to the tissue.  By transmitting the proper wave to the fatty tissue, the fat cells vibrate and enter into self-resonance.  Electrical current fields are produced in CELLSOUND treatments through 4 electrodes that are attached to the client’s body in the desired treatment area.  The electrodes create an electrical field on the skin surface to drive ions through the skin.  The body’s response to electrical current may cause changes in the physiological functions that occur at various levels.  The electrodes create muscle contraction through nerve or muscle stimulation and create changes in cell membrane permeability.

Where does the fat go?

Each of the two technologies has a different effect on the adipose cell:

1) Ultrasound:  creates the resonance effect, releasing free fatty acids without disrupting the cell membrane.

2) Electric currents fields:  assist the free fatty acids to leave the cell without breaking the cell membrane by increasing the cell’s membrane permeability, thus becoming available for the body to use.

The free fatty acids that were released from the fat cells enter the interstitial tissue. The liver then sends receptors to the free fatty acids and transports them back to the liver via the blood stream. They are now available for the body to be used as energy. Fluids are also released from the fat cell at this time and are released from the body via the lymphatic system.  The amount of free fatty acids released during a 40 minute CELLSOUND treatment is available for the body to use for the subsequent 48 hours.

When fat cells are not as full after a CellSound treatment, the fat cells become smaller.  When they are smaller, they are drifted away from one another.  The pressure on the capillaries is reduced, leading to a reduction in the appearance of cellulite.  The blood flow through these capillaries is improved, and the metabolism rate is increased, enabling an improvement in skin texture and regeneration.

As our body uses energy continuously from simple breathing to performing more intense physical activity, it is up to the client to set the result desired.  A test was conducted in the “Hadassah” Medical center, the largest medical center in the Middle East, in preparation for the medical study on this subject.  The cardio-pulmonary exercise testing checks the body’s use of energy, carbohydrates and glucose or fat, while the body is active (during exercise).  The conclusion was that a single CellSound treatment increases the utilization of fat in moderate-intensity exercise by 52% compared with pre-treatment values. “CellSound treatment causes an amplification of fat oxidation during exercise compared to the natural state.”

Since the body’s energy demand is constant, it naturally uses the most available energy sources.  The free fatty acids released during the treatment are the most available energy source. Therefore, the free fatty acids that were released from the fat cells during the CellSound treatment are being used first.  The result is a reduction in circumference in the treated area.  Fat cells become leaky, and the fatty acids are bioavailable for use.  Limiting alcohol and sugar intake, exercise, and drinking more water following treatment may further help to flush the fatty acids from the body.   You may also notice increased bowel movements for 2-3 days.

How to prepare for your treatment:

1. No topical numbing is needed, as the treatment is tolerable. There are no needles or injections.
2. Shave any hair in the areas to be treated (lower half sideburns, jawline for face / neck, abdominal hair).
3. No makeup please. Arrive to clinic with a clean face, and bathe to remove any body oils prior to the treatment.
4. We will ask you to wash your face to remove any lotions or oils for face & neck treatments.
5. Wear loose, comfortable clothing that you don’t mind getting ultrasound gel on.
6. We will take standardized photographs in your underwear for body treatments.
7. Consider bringing hair clips or a headband, if you have bangs.
8. If you have any hyaluronic acid filler or chin implants, please advise us where they are placed. It is best not to do treatments directly over these areas (4 months after filler).
9. We will take an extensive medical history as certain conditions are contra-indicated for this treatment (e.g., seizure disorder), and medical clearance may be needed for patients who suffer from arrhythmia or other serious conditions.

During treatment

1. Expect for your abdominal muscle to contract involuntarily (feels like a baby moving inside of you) during body treatments, and for uncontrollable facial contractions (spasms) during face / neck treatments.
2. The contractions may feel awkward, but they are tolerable. You may feel like you are making an “ugly face.”
3. The ultrasound applicator must feel warm (about 4-5 on a scale of 1-10), in order for the treatment to work. The body adapts to the non-focused ultrasound energy and increases the skin impedance.  We will be checking in with you often to increase the electrode settings (increased contractions), which allows the ultrasound to target the tissue.
4. Let us know if the ultrasound stops feeling warm, so we can increase the electrode settings to ensure best results.
5. We will offer you a mouthguard or one dental roll on each side, to place between your upper and lower teeth. If you experience any dental pain, we can reposition the electrodes & apply more gauze between the gums and the teeth.

After Treatment

1. Take your time to get up slowly from the horizontal position, especially if you have low blood pressure.
2. Drink 1 gallon of water a day to help flush out all the fat and any toxins.
3. Weigh yourself every day during the treatment series, which is a good idea for permanent weight loss. Patients who gain more than 5 pounds after their CellSound series may not maintain their results.
4. Avoid alcohol, sugar, processed foods, & simple carbohydrates during the treatment series and for 6 weeks to reduce sugar dependency. Watch That Sugar Film, available on Amazon Prime and read The Obesity Code by Jason Fung.
5. Get adequate sleep. Adults need about 8 hours of sleep a day, and cortisol levels will increase with sleep deprivation. Cortisol causes people to store abdominal fat.
6. Consider creating a food & exercise plan to maintain your results long-term. We can refer you to a nutritionist.
7. There is essentially no down-time in terms of skin care or skin reaction. You can go right back to your daily activities.

Mole & Tattoo Removal

Laser removal of moles and tattoos of all colors utilizes a non-invasive procedure with the Lutronic Hollywood Spectra nano-acoustic laser.  The number of treatments to remove a mole or tattoo depends on the size, location, depth and color of the mole or tattoo. 

Laser treatments selectively target the ink used with specific wavelengths and are safer than other treatment options for removing a mole or a tattoo.  Options such as surgical excision, dermabrasion (or sanding) and deep chemical peels can be painful and may often result in scarring.  By emitting highly concentrated light energy that targets the tattoo ink pigments in the skin, the laser can specifically target certain colors, based on the wavelength of light energy chosen.  The pigment or tattoo ink absorbs the laser energy, causing the pigment to shatter into tiny particles that are gradually eliminated by the body. Laser tattoo removal offers a safe and effective method to fade or remove tattoos of various colors, sizes, and depths. 

Multiple treatment sessions spaced 4-8 weeks apart are required to achieve optimal results.  The number of sessions will vary depending on the size, color, location, depth, density of ink, age of the tattoo, and factors such as the individual’s skin type and propensity for scarring.  Spacing the treatment closer to 6-8 weeks apart allows your body to remove the maximum amount of tattoo pigment after each treatment.  Treatment should be performed 4-8 weeks apart in order to allow your body to remove the maximum amount of tattoo pigment after each treatment.  Each session takes about 15-30 minutes, so this can be achieved during the lunch hour.  Arriving early can allow the area to be numbed topically prior to having lidocaine injections.  

The laser is precisely directed at the tattoo, targeting the ink without damaging the surrounding skin.  During the treatment, some patients may feel a slight sting, like the snap of a rubber band on the skin.  Those who receive lidocaine injections should experience minimal discomfort during the lasering process.  

Immediately following treatment, the skin will have a temporary white discoloration, and the surrounding area may become reddish with associated swelling. This is normal and will disappear slowly over time. Results take about 5-6 weeks after treatment.  On occasion, a whitening of the skin may occur. However, normal skin tone should return within 6-12 months following your last treatment session.

Patient Instructions:  Before and After Laser & Light Procedures 

Pre-treatment instructions for all IPL and laser treatments include arriving to the clinic with a clean face, without makeup, lotions, or any cosmetics.  Avoid sun-exposure or tanning for 4 weeks prior to treatment, as tanned skin can shade blood vessels from the laser light, making treatment less effective and increasing the chance of discoloration.  Avoid over-the-counter and prescription retinoids (e.g., tretinoin) for a week prior to treatments.  High-risk patients such as those with melasma or significant history of post-inflammatory hyperpigmentation (brown spots appearing after trauma, bug bites or acne) should pre-treat with lightening creams such as compounded hydroquinone and kojic acid (available from the office), if not pregnant or breastfeeding, until the day of the procedure discourage melanocyte activation.  Bring a broad-brimmed hat to your procedure.  

Following the treatment, the laser treated area cannot be rubbed, scratched, or picked, or scarring can occur.  The crusted areas must be allowed to fall off naturally, and we recommend hydration with medical grade Dynasty Luxe Antioxidant cream and Epicutis post-recovery kit (oil cleanser, lipid serum, and masks).  Gentle cleansers must be used, with no rubbing or scrubbing the skin with a face cloth or sponge.  Gently pat the area dry rather than rubbing with a towel.  Avoid sun exposure and reapply sunscreen every two hours, even if driving or walking outdoors briefly. High-risk patients should continue to use hydroquinone or lightening creams immediately after treatment, including the evening of the treatment. For resurfacing procedures, avoid contact sports or excessive sweating during the one week healing period, while the treated area has reddish or purple discoloration.  Avoid wearing pressure from tight clothing if treatment is on the body.  

Aesthetician, Microdermabrasion, Microneedling, and Extraction Services

In-office acne extractions are a simple an effective way to reduce the appearance of microcysts such as milia.  We also offer Diamond Glow acne facials offering the gentle removal of non-inflamed acne lesions like blackheads and whiteheads. Through specialized tools, sebum and debris are gently extracted from pores, helping unclog them and prevent progression to more severe forms of acne.

During the procedure, thorough cleansing and preparation ensure hygiene, and sterile instruments are used to apply controlled pressure around pores for safe extraction. Incorporating steam and specialized serum facilitate the process.  Microdermabrasion and microneedling are other options to help smooth the skin.  Our experienced providers take special care to prevent harm, scarring, and rebound flaring of the skin or transient hyperpigmentation.  Topical numbing is recommended for microneedling, which is offered individually or a package of treatments for optimal benefit.  Following your facial and skin assessment, we will tailor a customized skin care regimen, offer an appointment for prescxription medications, and discuss lifestyle adjustments so that you can achieve the healthy and radiant skin that you want.  

Cyst Drainage and Cyst Removal

Cyst are sacs that develop underneath the skin surface that can fill with skin breakdown products, become irritated and inflamed, or explode.  Cyst drainage or aspiration is a medical procedure to relieve the contents of the cyst.  However, the cyst may recur if the entire sac is not removed under local anesthesia excision.  Cysts can occur due to the nature of the hair follicle, structure of the surrounding glands that become blocked, excessive friction, or hereditary factors.  If a cyst grows rapidly, rubs on clothing, causes discomfort, or trigger other concerns, these procedures may be covered by medical insurance.  Other less invasive options for treatment include a dilute cortisone injection, oral anti-inflammatory medication or antibiotics, and warm compresses.  Complete excision is a 30-minute procedure requiring sutures and should be scheduled accordingly.

Skin Tag Removal (Scissor, Cryotherapy, and Electrodessication)

Medical cryotherapy is far more effective than store-bought wart freezing sprays, as our liquid nitrogen is much colder with a boiling point of about −196 °C.  Cryotherapy, or cryosurgery, uses these freezing temperatures to create a local, targeted frostbite reaction to destroy abnormal tissue by inducing cell death, while preserving surrounding healthy tissues. Our experienced clinicians understand the art of applying this non-invasive approach in the proper way to effectively treat a variety of concerns such as skin tags, warts, seborrheic keratoses, pre-cancerous actinic keratoses, and other annoying growths.  

Cryotherapy is particularly effective for treating superficial, smaller, and well-defined lesions, as the recovery period is often 1-2 weeks for facial lesions, and 2-4 weeks off the face.  The temporary cold sensations and mild discomfort are brief and subside quickly.  A blister or crusted area will likely form, and these will shed off once the healing skin underneath surfaces.  Not picking these healing blisters or crusted areas is essential for proper healing, as it avoiding sun exposure on these spots, as post-inflammatory pigmentation, infection, or scarring can occur.  Multiple sessions might be required, depending on the size of the lesion, and treating the area sooner rather than later allows for more effective resolution.  Depending on the symptoms of the lesion, this is a procedure that may be covered by insurance.

Skin tags can be removed cosmetically with scissor removal, after the area is cleansed and prepared under sterile conditions, with or without lidocaine and using chemical or electrical cautery to stop any bleeding. Electrodessciation, or electrocautery, is an effective tool using a low setting of a high-frequency of electrical current to treat various benign skin conditions such as sebaceous hyperplasia (dilated oil glands), dark black spots such as dermatosis papulosa nigra (DPN), seborrheic keratoses, syringomas, hemagniomas or red vascular lesions, and other growths.  Electrodessication can precisely pinpoint the lesion and not hurt surrounding, healthy skin, resulting minimal pain, downtime, and scarring.  Strict sunlight avoidance and not picking the healing wound is necessary, as described above, following cryotherapy or chemical peels.

The treated area is numbed with local anesthesia if a large or sensitive area will be treated. Then, it is cleansed and the electrode is carefully applied to the targeted area to destroy the lesion.  Depending on the number of lesions treated, there can be some social downtime, but these lesions can be covered with makeup or concealer.  This procedure will not effectively treat moles.   

Cortisone shots 

Inflamed acne “zits,” pustules and cysts can be treated effectively with a dilute steroid, with resolution often within hours.  These treatments can be useful prior to a celebration (gala, wedding, prom), or an important audition or meeting.  Cortisone shots can also be used to treat painful or itchy hypertrophic scars or keloids. We will often mix the cortisone with other medications to reduce the possible complication of atrophy or “dents” in the skin surface.  These depressions occur infrequently and almost always resolve over time.  Areas of focal hair loss, due to alopecia areata, or inflammatory hair loss due to frontal fibrosing alopecia and discoid alopecia, can be treated with monthly, dilute steroid injections to prevent spread.  Treating this condition rapidly avoids “burnout” of the follicles, which can lead to permanent hair loss and scarring.  Cysts and sinus tracts from hidradenitis suppurativa or inflamed scalp lesions from folliculitis decalvans or dissecting cellulitis can also be treated rapidly with injections.  Individual psoriasis lesions or nail psoriasis can be treated with steroid shots also.

Intramuscular injections in the buttock, thigh, or arm, are also a common way to treat intense conditions quickly, such as rapid and diffuse hair loss, or full-body rash due to eczema, psoriasis, or allergic reactions, as corticosteroids reduce inflammation and alleviate associated symptoms by toning down immune responses.  As these shots distribute corticosteroids throughout the body by being release slowly from the muscle, large areas can be treated effectively for 1-3 months, with far fewer side effects than taking oral steroids, which are notorious for having a rebound effect.   Patients who suffer from high blood pressure, diabetes, insulin resistance, severe osteoporosis, or active infections should avoid having cortisone shots.

Acne Scar Reduction

Although preventing acne scars with early intervention is ideal, many people suffer from acne scars which are worsened by picking, sun exposure, and severe inflammatory and cystic acne.  Reducing the appearance of acne scars can be a very frustrating, time-consuming, painful and expensive process.  However, the benefits of gaining a healthier self-image and much improved confidence can far outweigh the costs of acne scar treatments.   

Most treatments can be categorized as topical (medical grade skin care products and prescription medications), oral (anti-inflammatory, antibiotics, hormonal, and Accutane® (Isotretinoin), or procedural (aesthetician microdermabrasion and Diamond Glow facials, pigmentary improvements with peels, intense pulse light with Lumecca, and textural improvements with a series of microneedling, Morpheus RF microneedling, resurfacing with Hollywood Spectra, TCA cross to heal ice pick scars, CellSound ultrasound collagen remodeling), and procedural such as punch elevation, subcision to release rolling scars, or excision of acne cysts, or scar revision for extensive scarring.

Skin Care & Prescription Medications

Chemical Peels

Chemical peels can address a variety of concerns including blemishes, scaly skin, blotchy skin, pigmentation, wrinkles, and acne scars.  We offer gentle peels with minimal downtime to medium depth chemical peels that can effectively treat pre-cancerous skin and wrinkles especially around the lips or eyelid area.  

By expertly applying a chemical solution and occasionally layering different mixtures, this procedure gently removes the outermost layers of the skin and exfoliate the unwanted skin.  Following the peel, the new skin is more youthful, smoother, and with fewer wrinkles and blemishes.  revealing a smoother, more youthful complexion. Sun damage can be slowly erased with a series of peels, which are a comfortable and simple process with our protocol. More intense medium depth peels can require 7-10 days of dry skin with peak changes during days 3-6.  As this new skin is revealed, it is important to protect it from the sun by wearing a hat and reapplying sunscreen.  You are welcome to use makeup to mask the appearance as you recover.

Chemical peels for acne scars range can be performed to target depth ranging from superficial and medium to full thickness with 100% TCA (see TCA CROSS).  Superficial and gentle peels such as glycolic acid, salicylic acid, and Jessner peels can benefit those struggling with mild to moderate acne scars or the pigmentation effects from the scars.  These superfical peel procedure require less downtime with minimal peeling and flaking but can promote effective exfoliation.  Promoting rapid skin turnover leads to a faster removal of brown spots and improves uneven skin texture by keeping pores clean and reducing the appearance of blackheads.  A series of treatments (5 or more) may be needed to achieve the desired results.

More severe scarring would benefit from a TCA peel which promotes more skin peeling (e.g., molting like a snake) and crusting with coffee-grains like darkening of brown spots.  People who undergoing a TCA peel may have social downtime and want to recover from home due to significant scaling and need to avoid sun exposure, in order to allow the skin to heal.  These results tend to be more dramatic, and TCA can be combined with Jessner and other variations to achieve medium depth peeling with less risk and downtime.   Fewer treatments may be needed, with more visible improvement after just one treatment.

TCA Cross for scars

Highly effective and minimally invasive, TCA CROSS (Chemical Reconstruction of Skin Scars) is a precise, controlled, and specialized technique used to target ice-pick and atrophic acne scars involves targeted application of trichloroacetic acid (TCA) to the individual scar indentations or “pits.” 100% TCA stimulates collagen production by creating a micro wound, thereby helping promote skin remodeling, resulting in improved texture and reduced appearance of the scars. By targeting the scar and not surrounding skin, the indentations heal and elevate to a more similar level as the healthy skin. 

TCA CROSS can improve the appearance of deep and stubborn scars and can be used alone or in combination with other scar revision techniques such subcision, medium depth peels, laser resurfacing, and IPL. The procedure is generally well-tolerated and has a relatively short recovery time, with minimal discomfort and downtime.  The main risk of the procedure is post-inflammatory hyperpigmentation and scarring if the healing wound is picked or scratched off, before it is ready to fall off on its own.
